Unannounced monitoring inspection was conducted beginning at 9:30 and ending at 11:00. There were no children in care when the LI arrived but one child, age 18 months arrived during the inspection. Child was observed playing with toys and cuddling with the provider. Reviewed children's files and family files - see violation notice The provider does not give medication. Standard #: 22VAC40-111-830-B Description: Based on a review of the Emergency drill form, it was determined that there was only one shelter in place drill this year. Evidence: There was one drill in February 2016. No second drill was done as required by the standards. Plan of Correction: Two shelter in place drills will be done each year.
Standard #: 22VAC40-111-60-B Description: Based on a review of 2 children's files, it was determined that the files were not complete. Evidence: File A is missing signed physical, address and phone number for one parent, Doctor's phone number, Medical insurance number; Medication Policy, Emergency Preparedness plan. File B is missing a physical. Plan of Correction: All information will be on file.
Standard #: 22VAC40-111-430-A Description: Based on observation it was determined that there is no Gate at the steps from the living room. Evidence: There is an 18 month old child in care. She is often in care in the living room and has access to open steps to the upper level and to the basement level. Plan of Correction: There will be gates put up .
Standard #: 22VAC40-191-50-A-1-a-2 Description: Based on a review of past violations, it was determined that one family member did not have a current Child Protective Services check. Evidence: Family member #1 - has a CPS check that expired over a year ago, Plan of Correction: We have tried to redo. Will now send another form.
