Inspector Discussed: maintaining individual records
An unannounced monitoring inspection was conducted today from approximately 2:30pm-3:55pm. There were 6 children(12 points) directly supervised by 2 staff. The home, 2 staff records, 4 children?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed during rest time. There was an abundant supply of books, toys, and materials for the children. The home was clean and organized. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-111-60-B Description: Based on review of children record all required information was not maintained in individual file. Evidence: During inspection child #3 was missing: 1 of 2 emergency contacts and health policy number Plan of Correction: Provider had a siblings file with information and will transfer information and have parent confirm.
Standard #: 22VAC40-111-180-A Description: Based on review of staff files Tuberculosis tests or screening were not maintained every two years. Evidence : During inspection the provider and assistant did not have up to date TB tests or screenings. Plan of Correction: I will go get it done
