63.2 Liability Insurance Disclosure

An unannounced full renewal inspection was conducted today. The provider was caring for 3 children, totaling 6 points. The children were observed finishing lunch, playing with toys, and preparing for their afternoon nap. This inspection was completed between approximately 11:00 am and 12:00 pm. Standard #: 22VAC40-111-60-B
Description: Based on a review of children's records, it was determined that one record is not complete. Evidence: Child record #4 does not include the information for a second emergency contact person.

Plan of Correction: The parents said they do not know anyone to list as the second emergency contact. I will ask them again and make sure they provide the information.

Standard #: 22VAC40-111-240-A
Description: Based on observations, it was determined that one area of the home has not been maintained in a safe and operable condition. Evidence: The guardrail on the stairs leading to the basement is very loose and is no longer securely attached to the wall. The children use these stairs daily to go downstairs for their nap and for afternoon play time.

Plan of Correction: The guardrail will be repaired.

Description: Based on observations, it was determined that the bathroom is not equipped as required. Evidence: There are no paper towels available for use in the children's bathroom.

Plan of Correction: I will keep a roll of paper towels in the bathroom.
