Technical Assistance:
Discussion held regarding improved ways of documenting annual staff training.

Comments:
Today an unannounced monitoring inspection was conducted with Provider. This visit took place in the morning between 8:45am and 10:45am. There were 12 children present with the Provider and 1 assistant(s) for a total of 33 points. Provider was found exceeding the supervision guidelines. The children were observed having free play which consisted of playing with trains, blocks and hand puppets. Observation of diapering, snack time, hand washing of both staff and children and morning sing-a-longs. Positive interactions were observed during this inspection. A complete inspection of the physical plant used for programming, 5 children and 3 staff records were reviewed, fire drill log, menu, attendance and emergency procedures were observed during this inspection. Violations:
Standard #: 22VAC40-111-240-A
Description: Based on observation of physical plant, some areas not maintained in good repair. Evidence: There were two cribs in the infant room with crib rails that had the finish worn off exposing the wood from biting.

Plan of Correction: Provider will either refinish or replace the rails

Standard #: 22VAC40-111-350-E
Description: Based on observation of physical plant, water temperature exceeded the range of 105 and 120 degrees. Evidence: Faucet in common area bath had a water temperature of 145 degrees.

Plan of Correction: Will adjust the water temperature so it doesn't exceed 120 degrees.

Standard #: 22VAC40-111-570-A
Description: Based on observation of number of children in care, the 16 point allowance per caregiver was exceeded. Evidence: There were a total of 12 children in care, with a total of 33 points, with two staff present during part of the inspection.

Plan of Correction: Third caregiver was scheduled however arrived late. Provider routinely schedules two assistants.
