Technical Assistance:

The licensing inspector conducted an unannounced renewal inspection on Wednesday, January 25, 2017 from 9:15am to 10:34am. The children were observed eating breakfast upon the inspector?s arrival, and they were later observed playing with cars and other toys. Today, there were two children, totaling five points, present with one caregiver. The provider does not administer medication. The provider does not provide transportation. The provider does not have pets. Two children?s records, one caregivers? record and one household record were reviewed during the inspection. Last emergency drill: January 18, 2017 Shelter-in-place drills: February & August 2016 The violations from the previous inspections were checked for corrections. There were no repeat violations found during today?s inspection. The Department has not yet received a complete renewal application and annual fee from the provider. A complete renewal application and fee must be received by the Department prior to expiration of the license on February 5, 2017 or the license cannot be renewed. Standard #: 22VAC40-111-780-B
Description: Based on a review of children's records on 01/25/2017, the provider did not ensure that the emergency contact information required in 22 VAC 40-111-60 B 2 was reviewed annually with parents to ensure the information is correct. EVIDENCE: The most recent update of information in the record of Child #1 is dated 12/14/2015.

Plan of Correction: Will use the free online website to get 16 hours of training.

Standard #: 22VAC40-111-210-A
Description: Based on a review of caregiver records on 01/25/2017, the provider did not ensure that caregivers shall obtain a minimum of 16 clock hours of training annually in areas relevant to their job responsibilities. EVIDENCE: Caregiver #1 obtained nine hours of annual training for 2016.

Plan of Correction: Will get mom to sign the form this evening when she picks child up.

Standard #: 22VAC40-191-40-D-1-c
Description: Based on a review of caregiver records on 01/25/2017, the provider did not ensure that the licensee had a search of central registry before three years since the dates of the most recent central registry finding. EVIDENCE: The most recent central registry search in the record of Caregiver #1 is dated 1/10/2014.

Plan of Correction: Will send the paperwork out today.
