National fingerprint background check regulations go into effect on 9/30/17.
An unannounced monitoring inspection was conducted today. The provider and two assistant caregivers were caring for 10 children, totaling 25 points. The children were observed eating lunch, reading stories, and taking their afternoon nap. The provider served lasagna, mixed fresh fruit, and milk for lunch today. This inspection was completed between approximately 11:45 am and 12:45 pm. Thank you for meeting with me today. Standard #: 22VAC40-111-700-A Description: Based on a review of children's medications, it was determined that one medication was on site without an authorization form. Evidence: One child's non-prescription medication was on site without a medication authorization form completed by the parent. Plan of Correction: I will ask the parent to complete an authorization form.
Standard #: 22VAC40-111-120-B Description: Based on a review of caregiver records, it was determined that one record was not complete. Evidence: Caregiver record #2 does not include a second reference check. Plan of Correction: I called for the reference check but didn't document it. I will put the documentation in the file.
Standard #: 22VAC40-111-60-B Description: Based on a review of children's records, it was determined that two records were not complete. Evidence: Child record #1 and #2 do not include documentation to show that the provider viewed each child's original proof of identification. Plan of Correction: I will document that I viewed each child's original birth certificate.
Standard #: 22VAC40-111-240-A Description: Based on observations, it was determined that one piece of equipment has not been maintained in a clean and operable condition. Evidence: The diaper changing pad is ripped in two different places. Plan of Correction: I will replace the diaper changing pad.
