Discussed the following standards: operational responsibilities, care of children, and physical environment and equipment.
A complaint investigation was conducted today in response to allegation received by office regarding the physical environment and equipment. Based upon observations made, documentation reviewed, and interview, the preponderance of evidence supports the allegation; therefore, the complaint is deemed to be valid. Other violations, not related to the complaint, were also cited. The exit interview was conducted with the Provider. Standard #: 22VAC40-111-60-B Complaint related: No Description: Based upon observations made, a child's record was incomplete. Standard #: 22VAC40-111-90-A Complaint related: No Description: Based upon documentation reviewed and observations, the provider did not obtain documentation of a child's immunizations before the child attended the family day home. Standard #: 22VAC40-111-480-D Complaint related: Yes Description: Based upon observations made, equipment used by children was not used in accordance with the manufacturer's instructions. Evidence: On January 18, 2017, Child A (6 months) was placed in a car seat while she was swaddled by Caregiver #1, the car seat was placed in the living room near the front entrance of the family day home as the Caregiver waited for the parents to pick up the child. Child A was then buckled and another blanket was placed on top of the child. Car seats are manufactured for protection during travel and not as a replacement for chairs or sitting equipment. Standard #: 22VAC40-111-680-B Complaint related: No Description: Based upon observations made, caregivers did not ensure that children's hands were washed with liquid soap and warm running water after diapering. Evidence: A caregiver was observed during the diapering of Child B. After the caregiver was done diapering the child, Child B joined her friends to play in the children's playroom and the caregiver did not ensure her hands were washed.
