Comments:
An unannounced monitoring inspection was conducted beginning at 9:30 and ending at 11:15. There was one day care child in care today, age 2 years. The child was observed having snack, watching a children's program and coloring. Reviewed files for two children - see violation notice. Reviewed emergency escape drills and shelter in place - see violation notice Required information is posted. Violations:
Standard #: 22VAC40-111-830-A
Description: Based on a review of documentation and interview with the provider, it was determined that there was no documentation of an emergency escape drill in January. Evidence: The provider stated that she had done the evacuation drill but could not remember the date and did not document it.

Plan of Correction: The provider will document all drills as they are done.

Standard #: 22VAC40-111-100-A
Description: Based on a review of the files for 2 enrolled children, it was determined that 1 child did not have documentation of a physical examination. Evidence: Child #1 who started in care, in December, does not have a physical on file.

Plan of Correction: The provider called the parent during the inspection to explain what was needed.
