Comments:
An unannounced monitoring/renewal inspection was conducted today at 10:40am. There were 4 children (8 points) directly supervised by 1 staff. The home, 2 staff records, 2 children?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing freely. There was an abundant supply of books, toys, and materials for the children. The home was clean and organized. Areas of non-compliance are identified in this report. Standard #: 22VAC40-111-80-A
Description: Based on review, the provider failed to obtain within 7 business days of a child's first day of care, documentation and verification of the child's proof of identity. Evidence: Reviewed 2 children's records and found CH2 did not have documentation of proof of identity.

Description: Based on review, the provider failed to notify local law enforcement after not receiving documentation of a a child's proof of identity within 7 days after their first day of attendance. Evidence: Reviewed 2 children's records and found that CH2 did not have documentation of contacting law enforcement after failing to obtain documentation of proof of id from the parent(s) or CH2.

Description: Based on observation, the facility failed to ensure that the areas and furnishings of the home, inside and outside, are maintained in a clean, safe and operable condition. Evidence: Observed peeling paint (next to the crib) and chipping wall repair in the nap room behind the door.
