Comments:
An unannounced monitoring inspection was initiated today at 2:15pm. The physical plant, 3 caregiver/household member files, 4 children's files, caregiver training, child nutrition, children's activities, emergency procedures, drills and supplies were inspected today. There were 8 children, totaling 16 points, under the supervision of 3 caregivers. During the inspection, children were observed coloring pictures, enjoying free play and resting at nap time. The interactions between the children and caregivers were seen to be positive and constructive in nature. Today, the children were served macaroni and cheese, salad (w/ mixed vegetables) and milk. Standard #: 22VAC40-111-920
Description: Based on interview and file review, the posted menu does not list substituted foods that the children were served for lunch today. Evidence 1. The posted menu has "hot dogs w/ buns, carrots, bananas and milk" to be served for lunch today. The Provider stated that the children were served macaroni and cheese, salad and milk.

Plan of Correction: The Provider will make changes to the menu.

Standard #: 22VAC40-111-120-B
Description: Based on file review, 2 out of 3 caregiver's records do not have all required information on file. Evidence 1. Caregivers #2 and #3 do not have documentation that two or more references as to character and reputation as well as competency were checked before employment. The Provider has contacted two references, but did not document the findings.

Plan of Correction: The Provider will complete these.
