Technical Assistance:
Technical assistance was provided on the following items during the Inspection: 22VAC40-111-(6)-210 (Annual training); 22VAC40-111-(7)-500 (Outdoor play area and equipment)

An unannounced monitoring inspection was conducted on 2/21/17 from 10:40am - 12:15pm. At the time of entrance there were 9 children ages two years old to four years old (18 points) in care with two caregivers. A tour of the home was conducted and children were observed playing inside and outside with toys, eating lunch, doing science experiments, and during rest period. Records were reviewed for four children and four caregivers. There is no medication at the family day home. Emergency plan and emergency supplies were reviewed. Areas of non-compliance are identified on the violation notice, and were discussed during the exit interview.

Standard #: 22VAC40-111-830-B
Description: Based on a review of the emergency evacuation drill log, it was determined that the provider did not ensure that the Shelter-in-place procedures shall be practiced a minimum of twice per year. Evidence: 1. The emergency evacuation drill log did not have documentation for that any Shelter-in-place drills were completed in 2016. 2. The provider confirmed that that there were no shelter-in-place practice drills completed in 2016.

Plan of Correction: The provider responded: Effective immediately, shelter-in-place drills will be done 2x a year.

Standard #: 22VAC40-111-100-A
Description: Based on a review of four children's files, it was determined that the provider did not ensure that documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance is obtained. Evidence: The record for child #1 (date of enrollment 8/1/16) , present during the inspection, did not contain documentation of a physical examination.

Plan of Correction: The provider responded: Contacted mother who will provide a copy of physical.

Standard #: 22VAC40-111-60-B
Description: Based on a review of four children's records, it was determined that the provider did not ensure that they maintain and keep at the family day home a complete record for each child enrolled that contains all required information. Evidence: 1. The record for child #2, present during the inspection, did not contain the date of enrollment; the address, phone number, for the mother; the name, address and telephone number for the mother's place of employment; documentation for the second designated persons to call in case of an emergency if the parent cannot be reached; documentation to indicate whether or not he child had previously attended child care. 2. The record for child #4, present during the inspection, did not contain the documentation of the name and policy number of the child's medical insurance, and documentation to indicate whether the provider will administer medication.

Plan of Correction: The provider responded: Will update file and write in missing information. Mother has court order to have no contact with child at day care. Documentation in file. Contacted mother and will obtain provide policy number from parent. Medication authorization form reviewed and signed by parent.
