Comments:
An announced initial inspection was conducted today from 10:30am-12:00pm. There were 4 children(10 points) directly supervised by 2 staff. The home, 2 staff records, 1 children?s record, 1 household member?s records, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing freely. There was an abundant supply of books, toys, and materials for the children. The home was clean and organized. Areas of non-compliance are identified in the Violation Notice. Violations:
Standard #: 22VAC40-111-760-A-1
Description: Based on observation, the first aid kit was incomplete. Evidence: The first aid did not contain two triangular bandages or a first aid manual.

Plan of Correction: The provider will purchase the items needed to complete the first aid kit

Standard #: 22VAC40-111-60-B
Description: Based on record review, one child's file was incomplete. Evidence: Child #1's record did not contain the address or phone number of the parent's place of employment, or the name addresses and phone numbers of two persons to contact in the event of emergency.

Plan of Correction: Provider will obtain the information from the parents.

Standard #: 22VAC40-111-240-A
Description: Based on observation, some equipment in the family day home was not maintained in an operable condition. Evidence: Two tears were observed on the diaper changing surface preventing the area from being properly cleaned and sanitized.

Plan of Correction: The provider will purchase a new changing pad.

Standard #: 22VAC40-111-320-B
Description: Based on observation, not all accessible outlets had child-resistant protective coverings installed. Evidence: An outlet without a protective cover was observed to the left of mirror in the bathroom used by children.

Plan of Correction: Corrected during inspection.

Standard #: 22VAC40-111-520-C
Description: Based on observation, the crib used by the family day home did not meet the requirements for cribs. Evidence: The crib used by the family day home is a collapsible mesh sided crib.

Plan of Correction: The provider will purchase and assemble a crib which meets the requirements.

Standard #: 22VAC40-111-540-B
Description: Based on observation, the family day home had infant walkers in use. Evidence: An infant was observed playing in a wheeled infant walker.

Plan of Correction: Removed during inspection.
