Technical Assistance:
Discussed 16 hours of annual training needed by September. Also, only 2 shelter in place drills are needed annually but evacuation drills are needed each month.

An unannounced monitoring visit was conducted today from 9:20am to 10:45am. 2 children (7 points) and two caregivers were present. Ratios were in compliance. The physical space, first aid kit, emergency evacuation drills, 2 children's records, the provider and assistant records and Emergency Preparedness Plan were all reviewed. Required documentation was posted. The temperatures in the refrigerator and freezer were inspected due to a previous complaint. The refrigerator/freezer were in compliance. The children were observed playing and interacting positively with the caregivers. The space was clean, organized and the provider had an abundance of developmentally appropriate materials. Description: Based on review of the first aid kit, the provider did not have all required supplies. Evidence: The provider only had one triangular bandage.

Plan of Correction: I will purchase another triangular bandage.

Description: Based on record review, the provider did not obtain parent's signed acknowledgement of an annual review. Evidence: The emergency records for Child #1 were last reviewed on 11/1/15.

Plan of Correction: I will ask the parents to review the emergency documents and sign them today.

Description: Based on observation, the provider did not maintain the bathroom in a safe condition. Evidence: A drawer in the bathroom located next to the classroom had a missing front panel exposing screws.

Plan of Correction: I will remove the screws protruding from the drawer imediately.

Complaint related: No
Description: Based on observation, the diapering surface was not maintained with a nonabsorbent material. Evidence: The diaper changing pad was torn and exposed an absorbent material.

Plan of Correction: I will replace the changing pad immediately.
