54.1-3408 Provider must be MAT certified to administer prescription medication.

An unannounced monitoring visit was conducted today beginning at 11:40am. 9 children (30 points) were directly supervised by 2 caregivers. Ratios were in compliance. The physical space, evacuation drills, emergency supplies, menu, provider/assistant/household member records and 3 children's records were reviewed. Required documents were posted. Provider is MAT trained. No medications were on site today. Children were observed in free choice, working on puzzles, playing with cars and interacting with the caregivers. They were also observed washing hands and eating lunch. Today's menu consisted of pasta, peas, fresh fruit and milk or water. Areas of non-compliance are cited on the Violation Report. Description: Based on observation, the provider did not label bottles with the child's full name and date. Evidence: Bottles were placed in bins labeled with names in the refrigerator but each bottle was not individually labeled with a name or a date.

Plan of Correction: I will ask parents to label the bottles daily with names and dates.

Description: Based on record review, the provider did not maintain a copy of the tuberculosis (TB) test for the assistant. Evidence: The TB test for the assistant was not available for review.

Plan of Correction: She did it but I gave it back to her. I will get it back and send it to you.
