Technical Assistance:
Discussion was held on the topics of current enrollment and current staffing. Observation and discussion was also held on the status of Child #3. Staff was observed following up with the parent only to learn that this child was still stuck at school due to a school bus that had broken down. Good job to the Provider and her assistant in their ability to account for all children and to follow up when a child has not arrived at their usual time.

Comments:
The following inspection took place between the hours of 2 pm through 3:30 pm. Today, I met with the Provider on site at her Family Day Home (FDH) located in Ashburn, Virginia. At the start of this unannounced monitoring inspection there were a total of 2 children (6 points) with 2 caregivers; a third child was set to arrive late (see above). A total of 3 children?s files and 3 staff/household member?s files were reviewed. The children were engaged in the following: nap time, pick-up from school, and other organized activities. Standard #: 22VAC40-111-60-B
Description: Based on review, 1 of 3 children's files reviewed indicated that not all of the required information was contained within each child's record. Evidence - 1. On the date of inspection (03/08/2017), the policy number for the medical insurance of Child #1 was not available for review. 2. The name of policy was listed, but the section available to list the policy number was left blank.

Plan of Correction: The policy number shall be completed by the Parent of Child #1.

Standard #: 22VAC40-111-90-C
Description: Based on review, 1 of 3 children's file reviewed indicated that the Family Day Home (FDH) did not obtain documentation of additional immunizations for children (who were not exempt from the immunization requirements according to subsection B of this section) once every six months for children under the age of two years. Evidence - 1. On the date of inspection (03/08/2017), the following children had immunization pages that were outdated: Child #1 (immunization documentation dated - 07/20/2016), and Child #2 (immunization documentation dated - 07/16/2016). 2. Both children, based on the information on file, were under the age of two years on the date of inspection.

Plan of Correction: Updated immunization pages shall be obtained for both children and placed on file.
