Technical Assistance:
Consultation provided on standards 630 A ( 1-6) Behavioral guidance . Consultation provided on requirements for caregivers and the importance of one provider remaining within 16 points.

Comments:
An unannounced renewal study was conducted today with the provider who was present with 9 children for a total of 20 points. At the end of the inspection the substitute provider arrived with two schoolage children. The provider does not administer medication. Nine children's files were reviewed. For snack, the children were provided graham crackers and apple juice. Arrival time: 3:10 pm Departure time: 4:35 pm

Description: Based on a review of the monthly escape log and a conversation with the provider, it was determined that the provider has not conducted monthly escape drills. Evidence: There was no monthly escape drill for December 2016.

Description: Based on a review of nine children's files and a conversation with the provider, it was determined that a child's file did not have all of the required information. Evidence: Child A enrolled on 12/20/16 and there was no address for the second emergency contact.

Description: Based on a review of nine children's files and a conversation with the provider, it was determined that parents were not provided in writing all of the required information before the child's first day of attendance. Evidence: Child B enrolled on 10/3/13 and there was no documentation that parents were provided a copy of the provisions of the emergency preparedness and response plan nor was there documentation that parents were provided with a copy of the liability insurance declaration.

Description: Based on observation and conversations with the provider and a person functioning as a caregiver, it was determined that a person functioning as a caregiver was not at least 16 years of age. Evidence: Person A ( DOB 10/5/2001) was functioning as an assistant but is 15 years of age and not the required 16 years of age or older.

Description: Based on observation on the playground, it was determined that play equipment and its surroundings was not maintained in a safe and operable condition. Evidence: 1)The rug under the green and yellow play structure was torn and ripped and presented a tripping hazard. 2)The seesaw was broken and in disrepair.

Description: Based on observation it was determined that outside play equipment was placed on a hard surface. Evidence: 1)Seven of the swings on two swing sets were placed over dirt as the grass underneath had worn away. 2) At the end of the slide the grass had worn away and there was dirt at the end of the slide.

Description: Based on observation and conversation with the provider, it was determined that one caregiver exceeded 16 points . Evidence: The provider was present with 9 children for a total of 20 points. The second caregiver present was 15 years of age therefore could not be considered as a caregiver as caregivers must be at least 16 years of age.
