Comments:
The following inspection took place between the hours of 12pm and 1:45pm. Today, I met with the Provider on site at her Family Day Home located in Ashburn, Virginia. At the start of this unannounced monitoring inspection there were a total of 77 children (18 points) with provider and 1 assistant. A total of 5 children?s files and 3 staff and 1 household member files were reviewed. The children were all napping during this inspection. No medications at this time. Observation of physical plant provides ample space and equipment appropriate for the ages of children that are in care. Violations:
Standard #: 22VAC40-111-180-A
Description: Based on review of household records, subsequent TB screenings were not conducted. Evidence: Household #1's last TB screening was done in December 2014. Subsequent TB screenings are required every two years.

Plan of Correction: Household member will obtain a TB screening this Saturday.

Standard #: 22VAC40-80-120-E-2
Description: Based on observation of physical plant findings of the most recent inspection was not found. Evidence: Provider posts the most recent violation notice for a period of time and then files them away.

Plan of Correction: Provider will post today's inspection and will leave it posted.
