An unannounced SHSI inspection was conducted today, 3/21/17. A review of how to access the Appendix A and the Virginia Pre-Service training was also conducted. There are two children in care totaling four points with one caregiver. The inspection began 9:50 am ended 11:05 am. Should you have any questions please do not hesitate to call or e-mail. Standard #: VENDHOM-000-012-B Description: Based on review of staff/caregivers records the Virginia Pre-Service Training has not been completed within the first 90days of signing the vendor agreement. Evidence: 1. According to VACMS the vendor signed the agreement on 10/25/16. The provider or assistant have not completed the pre-service training. Plan of Correction: The provider will ensure that everyone gets this training.
Standard #: VENDHOM-000-012-F Description: Based on review of the caregivers records the assistant does not have first aid and CPR certification. Evidence: 1. Staff # 2 does not have first aid and CPR certification. Plan of Correction: The provider will check the community to see when one is available. The provider has first aid and CPR and will ensure that she is present at all times until the assistant is able to get this certification.

The provider does not have a written lock down procedure on file. Plan of Correction: The provider will ensure that the lock down procedures are written down.
