Comments:
An unannounced monitoring inspection was conducted on 3/21/17. Two children in care totaling four points with one caregiver present. Two children and staff's records reviewed for compliance. An exit interview was conducted. If you should have any questions please do not hesitate to call 276-608-2283. Inspection began: 9:50 am and ended 12:00 pm.

Standard #: 22VAC40-191-40-D-1-c
Description: Based on review of staff records the provider failed to have repeat background checks completed with three years of expiration. Evidence: 1. The central registry checks for staff 1 & 2 expired 12/17/16.

Plan of Correction: The provider mailed off the background checks request on 2/28/17.
