Comments:
Today an unannounced monitoring visit was conducted with the Provider's assistant. The Provider was not present during this visit. There were 4 children present with the Assistant for a total of 14 points within the supervision guidelines. The children were observed having morning free play, story-time and in transition for lunch. Positive interactions were observed during this inspection. A complete inspection of the physical plant, children and provider records, fire drill log, attendance, menu and emergency procedures. Violations:
Standard #: 22VAC40-111-50-A
Description: Based on a review of the family day home records, no written record of children in attendance. Evidence: Upon arrival, the family day home did not have written documentation of the attendance record for the children in care today.

Plan of Correction: Provider's assistant completed the attendance during this inspection.

Standard #: 22VAC40-111-320-E
Description: Based on observation of the physical plant, a portable electric heater was found in area accessible to children without a barrier. Evidence: Upon arrival, a portable electric heater was found in the play area with the children without a barrier to prevent the children from being able to touch it.

Plan of Correction: Provider's assistant unplugged the heater during this inspection and stated that it was only on to take the chill out of the room.
