Comments:
An unannounced monitoring visit was conducted today beginning at 3:00pm. 3 after school children were present (one child was the provider's child) with one caregiver. The caregiver's other child was napping. Ratios were in compliance. The physical space, Emergency Preparedness Plan, first aid kit, emergency supplies, attendance records, three children's records, provider and household member records were reviewed. Required documents were posted. The children were observed eating snack and engaged in free play. The space was clean, organized and contained an abundance of age appropriate materials. Violations:
Standard #: 22VAC40-111-60-A
Description: Based on record review and interview of provider, the provider did not obtain the name and policy number of medical insurance for each child in care. Evidence: No record of insurance name or the insurance policy number was written the records for children in care.

Plan of Correction: I will add that information to my registration forms.

Standard #: 22VAC40-111-180-A
Description: Based on record review, the provider did not obtain a Report of Tuberculosis Screening for self and adult household member. Evidence: The previous screening expired in 2014.

Plan of Correction: I did it. I will contact the doctor's office for another copy of the form.
