Today an unannounced monitoring/renewal inspection was conducted with Provider. This visit took place in the morning and afternoon between 10am and 12pm. There were 6 children present with the Provider for a total of 17 points which exceeds the maximum points allowed. The children were observed having free play, handwashing observed and having lunch which consisted of grilled chicken, broccoli and applesauce. Positive interactions were observed during this inspection. A complete inspection of the physical plant, children and staff records, fire drill log, menu, attendance and emergency procedures were observed during this inspection. Standard #: 22VAC40-111-100-A
Description: Based on child record review, provider did not obtain documentation of a physical exams for one child. Evidence: Child #3 began care on 6/13/2016 and does not have a physical on site, this was cited during the licensing inspection in November 2016 and remains uncorrected.

Plan of Correction: Provider will request the physical from the parent again. This request was made via texting the parent during the inspection. Parent stated that it was at home and will be provided tomorrow.

Standard #: 22VAC40-111-60-B
Description: Based on child record review, one child's record was incomplete. Evidence: Child #1's record is missing the following: proof of identity, immunizations and signed parent acknowledgement.

Plan of Correction: Provider will obtain these missing documentation from the parent.

Standard #: 22VAC40-111-210-A
Description: Based on staff record review, annual training was not obtained as required. Evidence: Staff #1 began programming year in December. Since December 2015 to December 2016, only 8 out of the required 16 hours of training was obtained. This remains uncorrected from the November 2016 licensing inspection.

Plan of Correction: Provider was provided some online trainings for resource and will obtain all required training by the next licensing inspection. Provider to begin obtaining training the end of the week.

Standard #: 22VAC40-111-230-A
Description: Based on staff record review, documentation of training was not found. Evidence: Staff #1 completed CPR/First Aid training on 2/18/17. Certification cards were not maintained.

Plan of Correction: Provider will obtain duplicate cards. Provider requested duplicate cards during the inspection and will maintain them in the record once received.

Standard #: 22VAC40-111-430-B
Description: Based on observation of physical plant, prohibited protective barrier was found. Evidence: At the top of the stairs leading to the basement, a pressure mounted gate was found. Provider currently has three children under the age of 2 in care.

Plan of Correction: The gate had a hard mounted device but broke. Provider will replace the broken hardware and to ensure the gate is properly installed.

Standard #: 22VAC40-111-570-A
Description: Based on children in care, provider exceeded the maximum number of points permitted. Evidence: When inspector arrived provider had a total of 6 children with a total of 17 points. A maximum of 16 points is permitted.

Plan of Correction: Provider was unaware of the 16 months to 23 months was equal to 3 points. Provider will abide by the 16 point maximum. Two of the children went to pre school during the inspection which reduced the points to 15 points.
