Technical Assistance:
Consultation was given on subsidy requirements and a visit being conducted by the Inspector in the next month to review regulations for subsidy. Swings that are under the outside deck should have resilient mats under them to provide protection.

An unannounced monitoring visit was conducted today. Two assistant caregivers were present with eight children for 35 points which is within ratios for supervision. The provider was away from the home for an appointment. Children were relaxed and engaged in puzzles, reading, infant play time, sleeping and am snack. Food is bought from home for lunch. Bottles were labeled and the refrigerator was at 38 degrees. No medication was reported to be on-site during the inspection. Space is well maintained and supplied with materials and equipment for children. Emergency drills were current as well as both assistant files and two children's records. Areas of non-compliance are identified in this report. Please sign and date the report and return it to me within 5 calendar days from date of receipt. Description: Based on record review, the written emergency plan has not been reviewed annually. Evidence: The written emergency plan was lasted signed 10/2015.

Standard #: 22VAC40-111-230-A
Description: Based on record review, documentation of annual training was not on-site for assistant #1. Evidence: Training certificates were not in the personnel file for the assistant and she stated they were at her home.

Plan of Correction: The assistant stated she will make copies and bring in the originals for her file.

Standard #: 22VAC40-111-320-B
Description: Based on observation, an outlet was uncovered by the cabinet in the small baby area. Evidence: An outlet was not covered by a child resistant protective cover.

Plan of Correction: Assistant replaced outlet cover during the inspection.

Standard #: 22VAC40-111-550
Description: Based on observation, a play pen was used as a designated sleeping area of a child. Evidence: A child had fallen asleep in a bouncy seat which was then placed inside a pack-n-play. Pack-n-plays may not be used as sleeping areas for children.

Plan of Correction: The assistant moved the child into the crib when it was pointed out by the Licensing Inspector.
