Discussion was held on the topics of current enrollment and current staffing.

The following inspection took place between the hours of 10 am through 12:30 pm. Today, I met with the Provider on site at her Family Day Home (FDH) located in Leesburg, Virginia. At the start of this unannounced monitoring inspection there were a total of 4 children (10 points) with 1 caregiver. A total of 9 children?s files and 2 staff/household member?s files were reviewed. The children were engaged in the following: games, toy play, drop of from school, movie time, lunch, nap time and other organized activities. If you should have any future questions, please call. Standard #: 22VAC40-111-830-C
Description: Based on review, documentation could not be located of the emergency evacuation and shelter-in-place drills. Evidence - 1. The drill log was not available for review. 2. The 2016 log was available. The Provider stated (during an interview conducted on the date of inspection - 04/04/2017) that the drill were conducted and recorded but the log could not be located.

Plan of Correction: Log will be found or recreated.

Standard #: 22VAC40-111-70-A
Description: Based on review, 1 of 9 children's files reviewed indicated that not all of the required documentation was contained within each child's record. Evidence - 1. On the date of inspection (04/04/2017), the Provisions of the Family Day Home's (FDH) Emergency Preparedness and Response Plan (EP&RP) was not available for review for Child #7. 2. Child #7 was in attendance on the date of inspection.

Plan of Correction: A completed form shall be obtained from the Parent's of Child #7 and placed on file.

Standard #: 63.2-1809.1-B
Description: Based on review, 1 of 9 children's files reviewed indicated that required documentation with regards to liability insurance was not available. Evidence - 1. On the date of inspection (04/04/2017), an acknowledgement in writing of furnishing the status of any liability insurance for the Family Day Home was not available for review for Child #7. 2. The start date for Child #7 was listed as 02/27/2017.

Plan of Correction: A signed Liability Insurance Declaration shall be reviewed and signed by the Parents of Child #7. Documentation shall then go on file.
