Technical assistance today included record keepings, physical environment and crib safety. For free in-home training, contact the Infant Network at 703 324- 8225.

Comments:
An unannounced renewal inspection was conducted today from 9:30 - 12:15 pm. There were nine children present for 36 points with a substitute provider and three assistants. I observed children playing outdoors, eating lunch and preparation for naptime. If you have any questions, please contact me at brenda.kuhlman@dss.virginia.gov

Violations:
Standard #: 22VAC40-111-120-B
Description: During today's review of staff and substitute records, no written references were available for some staff. Evidence: There were no written references available today for Substitute # 1. Assistant # 1 only had one reference, and Assistant # 2 did not have any references documented. (This standard was cited on 12/16 and 4/16)

Plan of Correction: They ill be here for inspection.

Standard #: 22VAC40-111-50-A
Description: During today's record review, it was noted that the attendance record did not meet the standard on recording each child in attendance today. Evidence: At 10:00, nine children were present but only seven had been listed on the attendance. All had been present over an hour.

Plan of Correction: We added the two other children. We will sign in children as they come in.

Standard #: 22VAC40-111-60-B
Description: During a review of children's records today, some required information was found to be incomplete. Evidence: There was only one emergency contact for Child # 1 (two are required.) There was incomplete health insurance information for Child # 2 and incomplete verification of proof of identity for Child # 2 and 3. (both in care longer than a week)

Plan of Correction: I will call the parents today to bring in the information.

Standard #: 22VAC40-111-140-D
Description: During today's inspection, there were no records available to confirm the substitute provider's hours as required when the provider is not present. Evidence: The form was not present, and the substitute reported she was not aware of the requirement to sign in and out to document hours in the absence of the provider.

Plan of Correction: The substitute will find the form on-line, print it and fill in her hours as she works this week.

Standard #: 22VAC40-111-200-C
Description: During a review of records today and staff confirmation, there was no documentation available to confirm that the substitute present today and three assistants had an orientation signed and dated by the provider and staff as required. (This was previously cited 4/25/16 and 12/16.)

Plan of Correction: The provider was not present today, and the substitute and assistants could not locate the written orientation. They report that they were provided with information concerning licensing standards and will have it documented when the provider returns next week.

Standard #: 22VAC40-111-240-A
Description: During today's inspection, some areas of the home were observed not to be maintained in a safe condition. Evidence: In the children's housekeeping area, a white bookcase had a protruding screw accessible on a shelf near the bottom.

Plan of Correction: It will be made flat or removed.

Standard #: 22VAC40-111-250-A
Description: During today's inspection, some hanging items were not kept out of reach of children under age five as required. Evidence: While children were playing outdoors, a large flat belt was observed to be hanging from a tree attached to a bell, accessible at child height. A long (4 ft.) rope was observed to be tied to a heavy push wagon, accessible on the grass.

Plan of Correction: The belt was to help the children ring the bell. It was put up away from the children. The rope was removed during the inspection.

Standard #: 22VAC40-111-280
Description: During today inspection, a hazardous material was observed to be in an unlocked area not inaccessible to children. Evidence: A closet in the children's bathroom was not latched today, and a spray cleaner was observed on the floor.

Plan of Correction: Corrected during inspection.

Standard #: 22VAC40-111-320-B
Description: During the inspection, an electrical outlet was observed to be uncovered at child height in the napping room. No covers were available to cover it today.

Plan of Correction: We will buy some.

Standard #: 22VAC40-111-320-G
Description: During a review of household paperwork, there was no documentation of an annual fireplace inspection. as required. Evidence: The last inspection for both fireplaces was dated 1/16.

Plan of Correction: She will take care of that when she gets back.

Standard #: 22VAC40-111-330-A
Description: During today's inspection, staff could not locate a landline telepone. Evidence: Staff did not know where it was located in the provider's absence.

Plan of Correction: This will be part of the orientation training when she gets back.

Standard #: 22VAC40-111-520-C
Description: During today's inspections, some cribs were observed to not meet standards as they were today. Evidence: One crib was observed with two mattresses inside, one on top of the other, not meeting the standard for the item to be used as directed by the manufacturer.

Plan of Correction: One mattress was removed during inspection.

Standard #: 22VAC40-111-530-B
Description: During today's inspection, two cribs were observed to have crib sheets had bottom covers that did not meet the standard for fitting. Evidence: Two crib sheets were observed to be too tight to allow the mattress to lay flat. As a result, the mattresses posed a hazard. (This was previously cited on 4/25/16)

Plan of Correction: We will get more sheets that fit correctly.

Standard #: 22VAC40-111-650-C
Description: During a review of records and through staff confirmation, there was no documentation that parents were given prior notice that a substitute provider would be caring for the children. Evidence: There is no documentation that the provider gave prior notice to parents before leaving the home day care with a substitute provider and assistants.

Plan of Correction: If she put it in writing, we don't know where it is. She will definitely do this in the future.
