Today an Announced initial visit was conducted with the New Provider. This visit took place in the morning between 10:30-1:00pm there were 2 children present with the Provider for a total of 5 points within the supervision guidelines. The children were observed having morning outdoor free play, playing with blocks, puzzles, 30 minutes of tv time, lunch and nap time. Lunch consisted of chicken nuggets, mac & cheese, oranges and milk. Positive interactions were observed during this inspection. A complete inspection of the physical plant, children and provider records, fire drill log, menu, attendance, emergency plan, first aid kit, and emergency procedures were observed during this inspection.
Standard #: 22VAC40-111-920 Description: Based on a review of the provider records, the provider did not have documentation of a menu for the currennt one-week period posted. Evidence: No documentation of a menu during this visit. Plan of Correction: Provider stated that she will create a menu and post it. Provider stated that she usually does her menu online.
Standard #: 22VAC40-111-50-A Description: Based on observation and interview with the Provider, the provider did not have documentation of written record of children in attendance each day. Evidence: Upon arrival & review of the provider records, there was no documentation of an attendance record. Plan of Correction: Provider stated that she will start to complete the attendance record each day.
Standard #: 22VAC40-111-60-B Description: Based on a review of the children records, the following information was missing: parents work phone number, medical insurance policy number. Evidence: Child #1-4's record did not include the child's medical insurance policy number. Child #3 & #4's record did not include the parents work number. Plan of Correction: Provider stated that she will ask the parents today for the missing information.
Standard #: 22VAC40-111-80-A Description: Based on review of the children's record, one record did not include proof of identity was verified within 7 days of child's first day of attendance. Evidence: Child #1's record did not include verification of proof of identity. Plan of Correction: Provider stated that she will request the birth certification to verify.
Standard #: 22VAC40-111-280 Description: Based on observation of the physical plant, cleaning agents were found accessible to children. Evidence: In the bathroom, cleaning agents were found under the sink unlocked and accessible to children. Plan of Correction: Provider stated that her husband will add a safety latch to the cabinet today.
Standard #: 22VAC40-111-290 Description: Based on observation of the physical plant, sharp objects were found accessible to children. Evidence: In the bathroom & kitchen cabinets, scissors and other sharp objects were found in a unlocked cabinet accessible to children. Plan of Correction: Provider stated that she will have her husband add a safety latch to all of the cabinets today.
