- Technical Assistance:
During inspection inspector informed provider the requirements for household members and assistants.
An unannounced renewal inspection was conducted today from approximately 10:00am-12:30pm. There were 5 children(14 points) directly supervised by provider. The home, 1 staff records, 3 children?s records, 1 household member?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing freely and eating snack. There was an adequate supply of books, toys, and materials for the children. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-111-830-A Description: Based on review of documentation and interview with provider evacuation procedures are not practiced monthly. Evidence: During inspection there was not documentation that an evacuation drill was conducted for the month of March. Plan of Correction: We will complete 2 drills in April.

Evidence: During inspection provider was missing: Current Background Checks ( Criminal History, Child Protective Services, Sworn Disclosure Statement) , Tuberculosis screening. Evidence: During inspection Child #1 was missing: Parents' work phone number, Health care policy number, notification of provider's policies, proof of identification and completed notification of liability insurance. Child #2 was missing: Parents' work phone number, Health care policy number, notification of provider's policies, proof of identification, completed notification of liability insurance and 2 of 2 emergency contacts. Child #3 was missing: first date of attendance , parents' work phone number, Health care policy number, notification of provider's policies, proof of identification, completed notification of liability insurance and 1 of 2 emergency contacts Plan of Correction: I can fix it, and show the parents.
Standard #: 22VAC40-111-160 Description: Based on review of documentation all required information was not maintained. Evidence: During inspection 14 year old household member did not have a complete CPS check on file. Plan of Correction: I will send it out
Standard #: 22VAC40-111-250-B Description: Based on inspection children were not protected from materials that could present a chocking hazard. Evidence: During inspection children were playing with toys that were less than 1-1/4 inches in diameter. Plan of Correction: Inspector removed from child's hand and mouth. Provider disposed of it.
