Technical Assistance:
Discussion was held on the topics of current enrollment, current staffing, and religious exemption for possible new enrollees.

Comments:
The following inspection took place between the hours of 2 pm through 4:30 pm. Today, I met with the Provider on site at her Family Day Home (FDH) located in the town of Leesburg, Virginia. At the start of this unannounced monitoring inspection there were a total of 9 children (24 points) with 3 caregivers. A total of 9 children?s files and 5 staff/household member?s files were reviewed. The children were engaged in the following: nap time, toy play, games, bathroom break, hand washing, large motor skills outside in the backyard, and other organized activities. Standard #: 22VAC40-111-100-A
Description: Based on review, 2 of 9 children's files reviewed indicated that the Provider did not obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance. Evidence - 1. On the date of inspection (04/13/2017), documentation of a physical examination was unavailable for review for the following children: Child #4, and Child #9. 2. The start date for each child was listed as - 08/08/2016 (for Child #4), and 02/14/2017 (for Child #9).

Plan of Correction: A physical shall be obtained for both children and placed on file.

Standard #: 22VAC40-111-90-C
Description: Based on review, 2 of 9 children's files reviewed indicated that the Family Day Home (FDH) did not obtain documentation of additional immunizations for a children (who were not exempt from the immunization requirements according to subsection B of this section) once every six months for children under the age of two years. Evidence - 1. On the date of inspection (04/13/2017), an updated immunization page was not available for review for: Child #3, and Child #4. 2. The most recent immunization documentation for Child #3 was dated 10/07/2016, and the most recent immunization documentation for Child #4 was dated 08/09/2016. 3. Based on the information within their files, both children were under the age of two years on the date of inspection.

Plan of Correction: Updated immunization pages shall be obtain for both children.
