Discussion was held on the topics of current enrollment, and current staffing. Discussion of the use of pressure gates within the Family Day Home. While there were two pressure gates present at the openings of stairways, all children present were over the age of 2 years and were developmentally ready to navigate stairways on their own. These same gates will need to be replaced as soon as younger children or those not able to navigate on their own are enrolled. Discussion on the need for additional documentation. The following forms were provided to the Family Day Home (FDH): ASSISTANT/SUBSTITUTE PROVIDER RECORD, CAREGIVER REFERENCE, DOCUMENTATION OF ASSISTANT AND SUBSTITUTE PROVIDER ORIENTATION TRAINING, GENERAL PERMISSION FOR REGULARLY SCHEDULED TRIPS, and SUBSTITUTE PROVIDER TIMESHEET.
The following inspection took place between the hours of 10 am through 1 pm. Today, I met with the Provider on site at her Family Day Home (FDH) located in Broadlands, Virginia. At the start of this announced initial inspection there were a total of 3 children (2 of 3 were residents) (5 points) with 1 caregiver. A total of only 1 child?s file and 5 staff/household member?s files were reviewed. The children were engaged in the following: art project, story time, free play, hand washing, bathroom breaks, large motor skilled activities outside on playground, games, lunch, and other organized activities. Standard #: 22VAC40-111-790 Description: Based on observation, not all emergency telephone numbers were posted. Evidence - 1. On the date of inspection (04/14/2017), while the telephone number for poison control was posted, the number "911" was not. 2. This was confirmed on the date of inspection by the Provider. Plan of Correction: Will post.
Standard #: 22VAC40-111-100-A Description: Based on review, the 1 child's file that was reviewed indicated that the provider did not obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance. Evidence - 1. On the date of inspection (04/14/2017), a physical examination was not available for review for Child #3. 2. Based on the information of file, the start date for Child #3 was listed as 11/17/2015. 3. Although there is a notarized Commonwealth of Virginia - Certification of Religious Exemption form on file for the immunization record, this form does not exempt one from the need for a physical examination to be on file. Plan of Correction: The Parent of Child #3 shall be contacted in order to obtain a physical for their child.
Standard #: 22VAC40-111-60-B Description: Based on review, the 1 child's file that was reviewed did not contain all of the required information. Evidence - 1. On the date of inspection (04/14/2017), the action to be taken, if Child #3 was exposed to the allergy listed, was not stated. 2. A field trip form was not on file for any walking field trip the Provider may make with the children off the premises. As an example, today at 11 am, the children and Provider went next door to the common area playground. Plan of Correction: All missing information/documentation shall be completed and placed on file.
