Technical Assistance:
No soft bedding of any kind shall be used under or around infants including but not limited to pillows and comforters. Emergency contacts should be someone other than the provider or parents.

Comments:
An unannounced monitoring inspection was conducted beginning at 9:30 a.m. and ending at 12:00 p.m. with the provider and assistant. There were five children in care ranging in age from twelve months to three years of age for a total of thirteen points. One child was observed having a breakfast of cereal and milk. Children were observed having free play and watching an age appropriate television program. Children had snack which was Club crackers and apple juice. Eight children's records and two caregiver records were reviewed. Please complete the plan of correction and date to be corrected by and return it no later than close of business on Friday, March 31, 2017. Standard #: 22VAC40-111-730-D
Description: Based on observation and interview with the caregivers, not all medication was kept in a locked place. Evidence: In an unlocked bedroom, there was medication stored unlocked.

Plan of Correction: This citation is denied. Provider has requested a supervisory review and discussion.

Standard #: 22VAC40-111-920
Description: Based on observation, the weekly meal and snack menu was not posted, provided to parents or placed in an area accessible to parents. Food substitutions were not listed. The weekly menu stated pancakes were for breakfast, however a child was observed having cereal and milk for breakfast.

Plan of Correction: Provide has posted weekly meal and snack menu for parents observation. Food substitutions shall be listed

Standard #: 22VAC40-111-100-A
Description: Based on review of documentation, the provider failed to obtain documentation of a physical exam within the first thirty days of attendance. Evidence: Child #3, enrolled 10/3/2016, did not have documentation of a physical in the record.

Plan of Correction: Provider will ensure that documentation of physical exam is obtained within the first 30 days of attendance.

Standard #: 22VAC40-111-50-A
Description: Based on review of documentation, attendance was not complete for March 27, 2017. Evidence: There were no attendance entries for today.

Plan of Correction: Attendance entries has been completed.

Standard #: 22VAC40-111-60-B
Description: Based on review of documentation, not all children's records were complete. Evidence: Child #1, enrolled 7/27/2015 did not have a second emergency contact other than the parent or guardian. Child #3, enrolled 10/3/2016, did not have a full address for a second emergency contact.

Plan of Correction: Provider will ensure that children's records are completed and placed in file.

Standard #: 22VAC40-111-80-A
Description: Based on review of documentation, the provider failed to obtain proof of age and identity. Evidence: Child #3, enrolled 10/3/2016 did not have proof of age and identity documented.

Plan of Correction: Provider has placed a request for child #3's birth record from parents

Standard #: 22VAC40-111-90-A
Description: Based on review of documentation, the provider failed to obtain documentation of immunizations prior to the first day of attendance. Evidence: Child #3, enrolled 10/3/2016, did not have documentation of immunizations in the record.

Plan of Correction: Children's immunization records shall be obtained prior to the childs first day of attendance. Child #3's record is complete.

Standard #: 22VAC40-111-240-A
Description: Based on observation, not all areas and equipment were maintained in a safe, operable condition. Evidence: There was a bucket of standing water outside of the entry door to the daycare entrance. There is a plastic crate in front of the door to the childcare that has a car battery inside. In the larger childcare area, there is a blue broken plastic tote that has sharp edges that create a scratching hazard.

Plan of Correction: This citation is denied. Provider has denied this citation and has requested a supervisory review and discussions.

Standard #: 22VAC40-111-280
Description: Based on observation, potentially poisonous substances were stored in an area accessible to children and near food. Evidence: The laundry room was not locked and there was a bottle of windshield wiper fluid on the floor accessible to children in care. In the larger childcare area, there was a bag of air freshener products stored in a drawer accessible to children in care.

Plan of Correction: This citation is denied. A supervisory review and discussions has been requested.
