Comments:
An unannounced monitoring inspection was conducted on April 14th 2017 between 10:30am and 12:15pm. There were 9 day care children and 2 provider children in the direct care of 3 staff members. Children were observed eating, having hands washed, and playing outside. A sampling of children's records, the staffing records, household records, the evacuation drills, the physical space, and supervision were observed and inspected. Violations:
Standard #: 22VAC40-111-90-C
Description: Based on record review, 1 of 3 children's records did not contain documentation of immunizations every 6 months for children under the age of 2. Evidence: Child #3 last had an immunization record on June 9th 2016. The child will not be 2 years of age until November 2017

Plan of Correction: I will talk with his mom today.

Standard #: 22VAC40-111-320-B
Description: Based on observation, not all electrical outlets contain child-resistant protective covers. Evidence: In the children's play room, there is an outlet that is not capped underneath the television.

Plan of Correction: I will go to the store today.

Standard #: 22VAC40-111-690-D
Description: Based on observation, the diapering surface was not nonabsorbent. Evidence: There is a 4 inch tear along one of the seams making the diapering surface absorbent when attempting to clean.

Plan of Correction: I will fix it for Monday.
