The licensing inspector completed an unannounced monitoring inspection on Friday, April 21, 2017 from approximately 12:00 pm to 1:58 pm to determine the provider's compliance with licensing standards. During this inspection, there were 11 children totaling 27 points being cared for by three staff including the provider. The children were observed by the licensing inspector sitting at the table eating their lunch. The children's lunch today consisted of pizza, corn, mixed fruit, green beans, and milk. The licensing inspector observed children interacting with their peers, caregivers, and the licensing inspector during their lunch time. The licensing inspector also interacted with the caregiver and the children when appropriate. Once the children's lunch time was completed, the licensing inspector completed observation of the children completing bathroom breaks, hand washing, and transitioning to go outdoors for playtime. During this inspection, the licensing inspector completed observation of the children participating in various activities to include freeze tag and simon says on the outdoor deck with staff. The licensing inspector observed the caregivers completing diapering during this inspection. The diapering station was inspected and deemed in compliance with licensing standards. Some of the children were observed indoors with caregivers playing with age appropriate toys and interacting with the caregivers. The caregivers were observed providing age appropriate supervision and addressing children's overall needs. During this inspection, the bathroom, kitchen, and designated classroom areas, and the playground were inspected. The designated area for childcare was deemed appropriate and equipped with age appropriate equipment and materials to meet the ages and stages of development of the children in care. The children were also observed by the licensing inspector completing hand washing when they transitioned indoors from outdoor play, and preparing for napping. The licensing inspector observed a parent pick up during this inspection as well. The inspector reviewed the required postings to include license, prior inspection summary, emergency contact information and breakfast and lunch menus. The first aid kit and emergency supplies, and fire and shelter-in-place drills were inspected and determined to be in compliance with the licensing standards. During this inspection, two children's, and two substitute providers/assistants' records were reviewed for compliance of standards. There are currently no medications being administered by providers. Standard #: 22VAC40-111-100-A Description: Based on review of children's records, the provider did not ensure documentation of a physical examination was obtained within 30 days after the first day of attendance.
Evidence: In the record of Child #2, there was no documentation of a physical examination found. Child#2 start date was recorded as 08/29/16. The provider shall obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.
Plan of Correction: The provider will talk with parent about obtaining documentation of child's physical today, request physician submit completed documentation to include in the record.
Standard #: 22VAC40-111-60-A Description: Based on review of children's records, the provider did not ensure an up to date record for each enrolled child was kept.
Evidence: During an interview with the provider and the caregivers present regarding a newly enrolled child, start date 04/04/17, at the family day home there was a request to review the record; it was determined that a complete record was not located on site. The child was not present during today's inspection, however, reportedly, the child had been in attendance since last week.
Plan of Correction: Provider will talk to child's parent about completing required documents for the record prior to return.
Standard #: 22VAC40-111-180-A Description: Based on review of caregiver's record, the provider did not ensure a current report of tuberculosis screening form was obtained for a caregiver every two years from the date of the first screening. Evidence: In Staff#1's record, the last tuberculosis screening form was dated 03/03/15, however, a current report of tuberculosis was due 03/17 and there was no updated record found in the record. The provider shall obtain for each caregiver a current report of tuberculosis screening form every two years from the date of the first screening. Plan of Correction: The caregiver will complete TB testing by 04/28/2017.
