Comments:
An unannounced monitoring inspection was conducted today, April 21, 2017 to determine the provider's compliance with licensing standards. The inspection was initiated at 3:25pm and concluded 5:30pm. In attendance were 5 children totaling 7 points being supervised by the provider. Children were observed playing with toys, interacting with provider, and departing for the evening. Today's snack consisted of a banana muffin and milk. Children's books, toys, and equipment were inspected and found to be age appropriate and in operable condition. The children's restroom was inspected with the required supplies present. All rooms of the home used by children were inspected with no hazards found.

The family day home had the required postings displayed on the bulletin board upon entering the home. The first aid kit, fire extinguisher, and emergency supplies were inspected. Two children's records and one provider record were inspected with the majority of the required information found. The following was also inspected: fire drill log, shelter-in-place drills, emergency preparedness plan, parent agreements, and other pertinent information.

Standard #: 22VAC40-111-170-A
Description: Based on the inspection of the provider's record, the required Tuberculosis test/screening has expired. Evidence: During the inspection of the provider's record, it was was discovered the Tuberculosis (TB) test/screening dated 12/18/14 expired 12/18/16. Licensing standards require TB test/screenings to be updated every two years from the date of the original.

Plan of Correction: Per provider: I will see if I can get to see the doctor next week and provide documentation for my file.
