Technical Assistance:
1. Reviewed age requirements for specific background checks. 2. Renewal applications will be sent by email now, unless requested otherwise. 3. Menu should be updated to reflect actual food served by the end of the day. 4. Recommend close supervision of children when they are climbing and sliding on play equipment.

Conducted an unannounced monitoring inspection 10:35am-12:45pm. Observed 8 children = 22 points with the Provider, Assistant #1, and Assistant #2. Ratios were in compliance. Children were playing with toys and play dough, climbing on play equipment, eating lunch, and taking naps during the inspection. Caregivers were playing with the children and promptly tending to their needs. Some children pack their own lunch, while others eat food from the Provider. Lunch served: peanut butter + jelly sandwiches, cucumbers, strawberries, and milk. The daycare areas of the home were found to be very clean, safeguarded appropriately, and well supplied with toys and equipment for the children. No medications reported to be on-site. Required documents were posted. The Provider's First Aid/CPR + MAT certifications were current. Record keeping violations found. Questions about this inspection may be directed to:

Standard #: 22VAC40-111-750-A
Description: Based on observations made and records reviewed, it was determined that diaper ointments were not properly labeled and there were no written parent authorizations. Evidence: 4 of 4 diaper ointment creams were not labeled with the child's name, and there was no written parent authorization for any.

Plan of Correction: We will label the diaper cream, and get parents to fill out the paperwork.

Standard #: 22VAC40-111-780-B
Description: Based on records reviewed, it was determined that 1 of 8 children's records did not include documentation of annual review and the subsequent parent signature. Evidence: Child #8 - The parent last signed the record on 9/27/15.

Plan of Correction: The parent will review and sign.

Standard #: 22VAC40-111-830-D
Description: Based on records reviewed, there was 0 of 2 shelter-in-place drills completed in the past 12 months.

Plan of Correction: We will write down when we do tornado drills two times a year.

Standard #: 22VAC40-111-50-C
Description: Based on observations made, it was determined that confidential information about children was posted in the home. Evidence: The Supplemental Information page of the 7/21/16 inspection, which had identifying information about children, on the bulletin board in the daycare area.

Plan of Correction: The paper was removed from the bulletin board during the inspection.

Standard #: 22VAC40-111-60-B
Description: Based on records reviewed, it was determined that 2 of 8 children's records did not include required information. Evidence: 1. Child #14 - The medical insurance policy number was not documented. 2. Child #15 - The medical insurance company and policy number were not documented.

Plan of Correction: I will ask parents for this information.

Standard #: 22VAC40-111-180-A
Description: Based on records reviewed, it was determined that caregivers and adults living in the home did not obtain a TB screening every 2 years. Evidence: 1. Provider - TB test on-file was dated 3/2/15. 2. Assistant #2 - TB test on-file was dated 3/7/15. 3. Adult #1 - TB test on-file was dated 4/2/15. 4. Adult #2 - TB test on-file was dated 4/1/15.

Plan of Correction: Everyone will get a new TB test.

Standard #: 22VAC40-111-490
Description: Based on observations made, it was determined that indoor slides and climbing equipment were over carpeting. Evidence: The slide climber and the caterpillar tunnel did not have a protective surface material in the fall zones. Children were observed sliding and climbing on the play climber, and on the top of the tunnel.

Plan of Correction: I will buy rubber floor matting and put it around the climbing equipment.

Standard #: 22VAC40-111-680-B
Description: Based on observations made, caregivers did not ensure that 4 of 8 children washed their hands with soap and water before eating lunch. Evidence: The children washed their hands, but were permitted to play with toys and puzzles for approximately 15 minutes before eating lunch.

Plan of Correction: We will wash hands just before eating from now on.
