Reminded provider that if she takes children off the FDH property she must have parents complete the field trip authorization form. Form provided during this inspection.
The following inspection took place between the hours of 8:45 am through 10:30. Today, I met with the Provider on site at the Family Day Home (FDH). At the start of this unannounced renewal inspection there were a total of 5 children (16 points) with 1 caregiver. A total of 5 children's files and 1 staff member's files were reviewed. The children were engaged in the following: toy play, movie time, snack, bathroom breaks, hand washing, and writing numbers. Areas of non-compliance are identified in the violation notice. Standard #: 22VAC40-111-960-E Description: Based on observation, there were 2 infants drinking bottles, neither of the bottles were labeled with the child's name and date. Evidence: When the Licensing Inspector arrived, child # 1 and #2 were drinking a bottle. Neither bottles were labeled or dated. Plan of Correction: I will label and date the bottles.
Standard #: 22VAC40-111-60-B Description: Based on record review, the child's record does not have contain all the required information. Evidence: Child #1 record does not have the medical policy number. Plan of Correction: I will ask the parent for the information.
