Technical Assistance:
Reviewed all the requirements with the provider, asked the provider to read the Appendix A to understand the requirements. If you have an assistant, the assistant must also have first aid and CPR and take the 10 hour Pre service training. You must add the lock down procedures to the emergency preparedness plan and practice a lock down drill once a year.

Comments:
An annual subsidy inspection was conducted today. The subsidy requirements were reviewed this date; allergy action plans, staff trainings, nutrition and food services, record keeping, hazardous substances, reporting injuries, emergency plans and transportation. If any violations were cited, they were assigned a risk assessment and discussed with the provider/director. The provider stated that she did not wish to continue with subsidy, she has no opening in her day care, she will email to cancel her contract. Standard #: VENDHOM-000-012-A
Description: Based on provider interview, the provider has not taken the Pre service training. Evidence: The provider said she has not taken the pre service training.

Plan of Correction: I do not want to have subsidy I will email to stop my contract. I will not do the training.

Standard #: VENDHOM-000-029-A
Description: Based on a review of the emergency preparedness plan, the provider has not addressed a plan for lockdown procedures in case of an intruder or violence near the premises. Evidence: The emergency preparedness plan was reviewed, the plan does not have procedures for a lockdown.

Plan of Correction: I don't want to do subsidy, I will email subsidy and stop my contract.
