An unannounced monitoring visit was conducted on April 27 2017 between 2:45pm and 4:00pm . There were 5 children in the direct care of a provider and an assistant. The children were observed napping, playing with a ball, and eating/drinking. The day care space was clean and organized. The children's records, staffing records, household records, evacuation drills, shelter in place drills, physical space, and supervision were observed and inspected. Standard #: 22VAC40-111-120-A
Description: Based on interview with the provider, an assistant on site does not have a record. Evidence: The provider stated that the previous assistant left and the new assistant started today.

Plan of Correction: I will hire an assistant starting soon with the complete paperwork.

Standard #: 22VAC40-111-540-A
Description: Based on observation, a chair that a child had fallen asleep in did not have the safety strap fastened. Evidence: In the napping room a child was not strapped into the seat she was using.

Plan of Correction: Anything that has a strap I will make sure to use.
