Standard 500C2: Please make sure that your play equipment is placed at least 6 feet away from outside edges of other play structures and obstacles such as a trees, fence and perimeter timbers. Please make sure you do a shelter-in-place drill this month.
An unannounced monitoring inspection was conducted today with the provider, who was present with six children for a total of 14 points. The children were eating their afternoon snack, which is brought from home, at the beginning of the inspection. There have been no new enrolled children since the last inspection. Three household member's files were reviewed as well as the vaccinations of the family pet. There are no medication on site and there have been no injuries/accidents documented. Arrival time: 2:15 pm Departure time: 3:20 pm
Standard #: 22VAC40-111-500-C Description: Based on observation, it was determined that the play equipment was placed over dirt. Play equipment shall not be placed over dirt or or other hard surfaces. Evidence: The yellow slide, see-saw, and three swings were placed over dirt. Plan of Correction: There was mulch underneath the play equipment but because of use and weather, the mulch has been replaced with dirt. I will replace the mulch.
