54.1-3408 Provider must be MAT certified to administer prescription medication.
An unannounced monitoring/renewal inspection was conducted today from 8:10am-9:30am. There were 4 children (15 points) directly supervised by 2 staff. The home, 2 staff records, 5 children?s records, 1 household member?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing freely and arriving to care. Diapering procedures were also observed. There was an abundant supply of books, toys, and materials for the children. Standard #: 22VAC40-111-830-B Description: Based on review, the provider failed to ensure that they are practicing shelter-in-place drills a minimum of twice per year. Evidence: Reviewed the emergency procedure log and found that there were no shelter-in-place drills conducted in the past 12 months. Plan of Correction: We will do one today and another one later on.
Standard #: 22VAC40-111-120-B Description: Based on review, the provider failed to obtain documentation of reference checks prior to the assistant's employment. Evidence: Reviewed the assistant's record and found that there were no reference checks on file. Plan of Correction: I will get refence checks from the assistant.
Standard #: 22VAC40-111-60-B Description: Based on review, the provider failed to ensure that children's records contained the required information. Evidence: Reviewed 5 children's records and found that... 1) CH1 did not have documentation of a medical insurance policy number. 2) CH2 did not have documentation of their first day of care. Plan of Correction: I will tell parents to write the information.
Standard #: 22VAC40-111-90-A Description: Based on review, the facility failed to ensure that they obtain documentation of immunization records before a child attends the family day home. Evidence: Reviewed 5 children's records and found that... 1) CH1 did not have documentation of an exemption. 2) CH3 did not have documentation of an immunization record. Plan of Correction: Spoke to parents and they will bring it in.
Standard #: 22VAC40-111-210-A Description: Based on review, the provider failed to obtain 16 annual clock hours of ongoing training. Evidence: Reviewed the provider's records and identified through the interview that they had not completed any annual trainings for the past year. Plan of Correction: I will do them.
Standard #: 22VAC40-111-320-B Description: Based on observation, the provider failed to ensure that electrical outlets are covered with child-resistant protective covers. Evidence: Observed 2 uncovered electrical outlets in the basement playroom, accessible to children under 5 years of age. Plan of Correction: I will cover them today.
