Comments:
11 Children were in care during the inspection total of 26 points. The provider and one assistant were in care. Children were observed during rest time and free play. No children were on medication. The age range of the children was four months to four years old. Inspection time 12:30pm to 2:35pm

Violations:
Standard #: 22VAC40-111-770
Description: Based on observation the emergency flash light and radio did not work. Evidence: During the inspection the flash light and radio did not work on batteries.

Plan of Correction: I will have a radio and flashlight that work.

Standard #: 22VAC40-111-830-C
Description: Based on review of the emergency evacuation drills not all were conducted monthly. Evidence: There was no documentation of a escape drill for April 2017.

Plan of Correction: I will conduct two escape practices in May.

Standard #: 22VAC40-111-120-B
Description: Based on review staff did not have references checked before employment. Evidence: Staff A did not have documentation of two references checked before the first day of employment.

Plan of Correction: I will check the references.

Standard #: 22VAC40-111-60-A
Description: Based on review not all children enrolled had a file. Evidence: Child A and B with with a start date of 4/4/17 were accepted into care with out the enrollment form or any required paper work.

Plan of Correction: Provider had parent email the enrollment forms for both children.

Standard #: 22VAC40-111-60-B
Description: Based on review of seven children's files not all were complete. Evidence: Child C did not have documentation of one emergency contact.

Plan of Correction: I will ask the parent for the missing information.

Standard #: 22VAC40-111-420-A
Description: Based on observation the room used for resting children did not have any light. Evidence: The room used for the children to rest was completely dark and the children could not be seen.

Plan of Correction: The provider turned on a night light making it possible to see the sleeping children.

Standard #: 22VAC40-111-540-B
Description: Based on observation there is a infant walker. Evidence: In the day care area there is an infant walker.

Plan of Correction: I will remove the infant walker from the day care.
