An unannounced monitoring inspection was initiated today at 2:30pm. The physical plant, 3 caregiver/household member files, 4 children's files, caregiver training, children's activities, emergency procedures, drills and supplies were inspected today. There were 5 children, totaling 12 points, under the supervision of the Provider. During the inspection, children were observed waking up from nap time and enjoying free play. The interactions between the children and caregivers were seen to be positive and constructive in nature. The children bring their lunches to the FDH each day that they are in care. The Provider is not certified in MAT (medication administration training) and there are no children with any medication needs. Standard #: 22VAC40-111-70-A Description: Based on file review, 2 out of 4 children do not have all required information on file. Evidence 1. Children #1 and #2 have "Information for Parents" in their file, but it is not signed by the parent. Plan of Correction: The Provider will ask the parents to sign this.
Standard #: 22VAC40-191-40-D-3-d Description: Based on file review, 3 out of 3 household members do not have a search of the central registry completed before three years since the last check was done. Evidence 1. Caregivers #1-3 have a search of the central registry completed on 3/27/14. Plan of Correction: The Provider will complete these again.
