An unannounced SHSI inspection was conducted May 4th 2017 between 11:45am and 2:00pm. There were 7 children in the direct care of 2 providers. Children were observed washing hands, eating lunch, playing outside, singing, and napping. Operational responsibilities, reports, staffing records, qualifications and training, the physical plant, and supervision were observed and inspected. Description: Based on interview with staff, Virginia Pre-Service Training had yet to be completed.

Plan of Correction: I wasn't aware of all of the changes because I was not notified. I will contact the person in charge. As soon as I get all of the information I will start working on the changes.

Standard #: VENDHOM-000-012-E-1
Description: Based on record review, 1 of 3 providers was not currently trained in cardiopulmonary resuscitation (CPR) and first aid. Evidence: Staff #3 certifications expired on April 29th 2017.

Plan of Correction: He will take it this weekend because there were no appointments this weekend.

Description: Based on observation, the provider does not have written lock down procedures.
