Technical Assistance:

The licensing inspector conducted an unannounced renewal inspection on Friday, May 12, 2017 from 11:00am to 1:20pm. The children were observed watching t.v. upon the inspector?s arrival. They were later observed listening to their caregiver read a book, playing with blocks, learning colors in sign language and eating lunch. Lunch consisted of: macaroni and cheese, ham, corn, fruit cocktail and milk. Today, there were seven children, totaling 14 points, present with two caregivers. The provider does not administer medication. The provider does provide transportation. The provider does have pets. Two children?s records, three caregivers? records and one household record were reviewed during the inspection. Last emergency drill: 5/4/2017 Last shelter-in-place drills: 03/01/2017, 5/5/2017 The violations from the previous inspections were checked for corrections. Standard #: 22VAC40-111-100-A
Description: Based on a review of children's records on 05/12/2017, the provider did not ensure that one of two records reviewed contained documentation of a physical examination prior to a child's attendance or within 30 days after the first day of attendance. EVIDENCE: The record of Child #1, who started at the family day home on 10/24/2016, did not have documentation of a completed physical examination.

Plan of Correction: Will let parents know at pick up today. Will ask to give to me by next week.

Standard #: 22VAC40-111-80-C
Description: Base d on a review of children's records and caregiver interview on 05/12/2017, the center did not ensure that one of the two records reviewed contained documentation of the verification of the child's age and identity. EVIDENCE: The record of Child #2 did not contain documentation that the child's proof of identity(id) had been viewed. Caregiver #1 stated that the proof of id had been viewed, but documentation was not in record.

Plan of Correction: Will text the parents today and ask to bring documentation today at pick up.
