Comments:
Today an unannounced monitoring visit was conducted with the Provider. This visit took place in the morning between 9:15am-11:45 there was 2 children present with the Provide for a total of 4 points within the supervision guidelines. The children were observed coloring, playing with blocks, taking morning nap, and in transition for lunch time. Positive interactions were observed during this inspection. A complete inspection of the physical plant, children and provider records, fire drill log, menu and emergency procedures were observed during this inspection. Standard #: 22VAC40-111-830-D
Description: Based on review of provider's monthly emergency response drills there no documentation of shelter in place drills. Evidence: No shelter in place drills to date were found documented.

Plan of Correction: Will document shelter in place drills beginning this month.

Standard #: 22VAC40-111-60-B
Description: Based on a review of the children records, one record did not include documentation of the name, address, phone number, two designated contacts, parents employer name, address and phone number. Evidence: Child #2's record did not have a complete child record to include documentation of the name, address, phone number, two designated contacts, parents employer name, address and phone number.

Plan of Correction: Provider stated that the parent did not provide the form to her yet.

Standard #: 22VAC40-111-320-B
Description: Based on observation of the physical plant, an electrical outlet was missing a protective cover and accessible to children under age five years of age. Evidence: Upon arrival, one outlet in the kitchen area of the basement was missing a protective cover and a child two years of age was present.

Plan of Correction: Provider covered during this inspection.
