54.1-3408 Provider must be MAT certified to administer prescription medication.
An unannounced renewal inspection was conducted with the provider beginning at 1:50 PM and ending at 3:31 PM. There were five children present during the inspection which included the provider's child. The ages of the children were 12 months, 21 months, 3, 3, and 5 years old for a total of 12 points. The children were observed finishing lunch which consisted of meatloaf, baked beans, bread, apples, and milk. The children were also observed hand washing and preparing for nap time. The emergency escape drills were reviewed and in compliance. There were no medications on site as the provider does not administer medication. The provider's First Aid and CPR certification is current. The first aid kit, flashlight, and radio were observed an in compliance. Standard #: 22VAC40-111-60-B Description: Based on a review of four children's records, it was determined that not all of the records were complete. Evidence: Child's A was missing the policy number of the health insurance. Plan of Correction: The provider stated that she had requested the missing information from the parent previously but will request it again.
Standard #: 22VAC40-111-240-A Description: Based on observation not all of the furnishings inside the home were maintained in a safe and operable condition. Evidence: One of the two plastic picnic tables in the play area had cracks on each side with jagged sharp edges exposed creating a scratching hazards. Plan of Correction: The provider stated that she would remove the picnic table and get a new table.
