Comments:
An unannounced complaint inspection was conducted on April 14, 2017 and May 10, 2017, in response to a complaint received in the Valley Licensing Office on April 4, 2017 concerning completion of injury reports. Interviews were conducted, observations made and documents/records were reviewed. The preponderance of the evidence did not support the allegation regarding lack of completing injury reports and the complaint is, therefore, not valid. Although not referenced in the original complaint, licensing standard 22 VAC 40-111-(11)-840-B (information required to be documented on injury reports) was found to be in violation. Refer to the violation notice for additional information. Please be reminded that it is the responsibility of the licensee to ensure ongoing compliance with the licensing standards. Description: Based on review of three injury reports and discussion with caregivers on May 10, 2017, the facility did not ensure the reports included all required information. Evidence: 1. Review of three injury reports, on this date, determined that the date and time of parent notification and the method of parent notification not been documented. 2. Caregiver #2 confirmed that injury reports dated 1/12/17, 3/30/17 and 5/05/17 did not included the date and time of parent notification and the method of parent notification.

Plan of Correction: The missing information was immediately added to the injury reports. In the future, complete information will be documented on all injury reports.
