Comments:
An unannounced renewal inspection was conducted today, May 23, 2017 to determine the provider's compliance with licensing standards. The inspection was initiated at 12:45pm and concluded at approximately 2:50pm. In attendance were 6 children totaling 16 points being supervised by the provider and assistant. Children were observed interacting with the provider, listening to a short story, playing with toys, and departing for the evening. Children's books, toys, and equipment were inspected and found to be age appropriate and in operable condition. All rooms of the two level home were inspected with no hazards found. Due to today's weather, the outdoor playground could not be inspected.

The family day home had the required postings displayed on the wall upon entering the home. The first aid kit, fire extinguisher, and emergency supplies were inspected with the required items present. Administrative records (including 3 children's and provider records) were inspected with violations found, see attached notice. The following was also inspected: fire drill/shelter-in-place log, emergency preparedness plan, parent agreements, and other pertinent information.

Violations:
Standard #: 22VAC40-111-840-B
Description: Based on the inspection of a child's injury report, the required signatures were not obtained. Evidence: During the review of an injury report dated 4/18/17, it was discovered the report only had the provider's signature. A second signature was not recorded on the form. Licensing standards require at least two signatures to be on the form to include the caregiver and parent signatures or two caregiver signatures.

Plan of Correction: Per provider: I will have the form updated with a second signature and will have two signatures in the future on injury reports.

Standard #: 22VAC40-111-60-B
Description: Based on the inspection of children's records, the required information was not listed.

Evidence:
During the inspection of three children's records, it was discovered the following information was missing:
Child #1 was missing the first date of attendance, parent's work address & telephone number, and the street address of the first emergency contact. Child #2 & #3 did not have the child's first day of attendance recorded. Licensing standards require this information to be listed in each child's record.

Plan of Correction: Per provider: I will have the parent's to update this information in each child's record.

Standard #: 22VAC40-111-70-A
Description: Based on the inspection of administrative records, the written information for parents did not contain the required information.

Evidence:
During the inspection of the written information for parents, it was discovered the presence of a pet or animal in the home was not listed. Licensing standards require this information to be listed for parents to review.

Plan of Correction: Per provider: I will update the agreement and have parents to sign off.
