An unannounced monitoring inspection was completed on May 25, 2017. The purpose of this visit was to measure the facility?s compliance with the licensing standards. There were 5 children in care, for a total of 13 points with two caregivers.
The children were observed reading stories, singing, dancing and coloring. Lunch for today was spaghetti and sauce with peas, blueberries, pears and milk. There is no medication on site. The provider is trained in medication administration. Both the provider and assistant are trained in CPR and first aid. The facility does not provide transportation. The last evacuation drill was conducted 5/15/17. The most recent shelter-in-place was conducted 2/28/17. Two caregiver, and two children?s records were reviewed.

Standard #: 22VAC40-111-120-B Description: Based on a review of caregiver records, the facility did not ensure that the record of a substitute caregiver contained verification of education. EVIDENCE: The record of caregiver #2, an assistant/substitute caregiver, did not contain verification of education. Plan of Correction: I will contact the school district to request the record or letter stating that her records are not available.
Standard #: 22VAC40-111-60-B Description: Based on a review of a sample of children's records, the facility did not ensure that a child's record contained written authorization for emergency medical care should an emergency occur and the parent cannot be located immediately. EVIDENCE: The record of child #1, did not contain written authorization for emergency medical care. Plan of Correction: I will make a copy for the parent to complete this afternoon.
Standard #: 22VAC40-111-540-B Description: Based on observations at the facility, the facility did not ensure that infant walkers were not used. EVIDENCE: The inspector observed an infant sitting in an infant walker. Plan of Correction: I will remove the two back wheels so that the chair will not be able to move.
