Discussion was held on the topics of current enrollment, current staffing, and signing subsidy forms two weeks past.
The following inspection took place between the hours of 9:30 am through 12 pm noon. Today, I met with the Provider on site at her Family Day Home (FDH) located in Centreville, Virginia. At the start of this unannounced renewal inspection there were a total of 7 children (20 points) with 2 caregivers. A total of 7 children?s files and 5 staff/household member?s files were reviewed. The children were engaged in the following: infant sleeping, toy play, music, bathroom breaks, hand washing, lunch, and other organized activities. Standard #: 22VAC40-111-60-B Description: Based on review, 5 of 6 children's files reviewed did not contain all of the required information within each child's record. Evidence - 1. On the date of inspection (05/25/2017), 1 of 2 emergency contacts listed did not have a phone number available for review for Child #1, and Child #2. 2. On the date of inspection, the following children's policy number for their medical insurance was not available for review: Child #3, Child #5, and Child #6. 3. This violation was cited previously during an inspection conducted on 02/23/2017. Plan of Correction: All missing information shall be obtained and placed on file.
