An unannounced monitoring inspection was conducted today. Observed 8 children (24 points) directly supervised by 2 caregivers. A sample of 2 children's records, and 2 caregiver records were reviewed. No medications were reported to be on site. The children were engaged in various activities, to include: free play, outdoor time, and infant care. Areas of non-compliance are identified in this report. The exit interview was conducted with the provider. Inspection conducted from approximately 9:45am to 10:15am. Description: Based upon documentation reviewed, the provider did not review the emergency plan at least annually and update the plan as needed. Evidence: The emergency plan was last updated/reviewed on 03/30/2016.

Description: Based upon observations, a snack brought from a child's home was not labeled. Evidence: A pouch of baby food was not labeled with the child's name.

Description: Based upon observations made, refrigerated bottles were not labeled. Evidence: A total of 4 bottles were not dated.

Description: Based upon observations made, children's records were not kept confidential. Evidence: The Supplemental Information page of the inspection conducted on August 30, 2016 was posted on the parent's board. This document states, "Information found on the Supplemental Information page is confidential and this document is not to be posted in the facility".
